Responsibilities

Conduct comprehensive OT assessments to determine functional limitations, goals, and best-fit interventions across ADLs and IADLs, mobility, cognition, and upper-extremity function.
Develop and implement individualized treatment plans aligned with patient goals, evidence-based practices, and interdisciplinary team input.
Deliver direct therapy, supervise upper-limb rehab, sensory integration, neuro-rehabilitation, adaptive equipment prescriptions, and home modification guidance to facilitate safe, independent participation.
Monitor progress with standardized measures, adjust plans accordingly, and document outcomes clearly in the electronic health record.
Provide caregiver and family education, discharge planning, and transition support to community-based services, ensuring continuity of care beyond the clinical setting.
Collaborate daily with physical therapists, speech-language pathologists, nurses, social workers, and other team members to optimize functional recovery.
Ensure safety, quality, and regulatory compliance; participate in team huddles, case conferences, and quality improvement initiatives.
