TravelNurseSource is collaborating with Cynet Health to recruit a qualified Case Manager RN in Orange, California. This position offers a weekly pay range of $2221 to $2467. The Case Manager will provide essential psychosocial support to individuals, families, and groups coping with chronic, acute, or terminal illnesses. The role involves advising family caregivers, providing patients with information and counseling, and making referrals for additional services. The Case Manager will report directly to the Supervisor of the Unit. The responsibilities of the Case Manager include performing admission, concurrent, and retrospective reviews on all patients to ensure accurate status and level of care determinations upon admission. It is crucial to obtain and document precertifications and authorizations for all admissions, surgeries, and procedures, following Medicare guidelines regarding utilization review. The Case Manager will also submit telephonic and/or fax notifications and clinical information, follow up with insurers for authorizations, and identify potentially high-risk patients for referral to Social Work. Effective clinical documentation is a key aspect of this role, as the Case Manager will conduct initial assessments to develop a comprehensive plan of care. They will ensure that the treatment plan is implemented, re-evaluated, and updated daily, facilitating effective communication among members of the treatment team. The Case Manager will coordinate meetings with families and the treatment team, arrange post-acute care services, and act as an advocate for the patient and family. Additionally, they will maintain accountability for facilitating clinical patient progression through a defined plan of care to achieve optimal outcomes. The role also involves identifying, documenting, and reporting quality, cost, and risk variances related to the patient's plan of care.
