About The Position

Tact Staff is seeking a travel nurse RN Case Manager specializing in Acute Care Case Management for a travel nursing job in Castro Valley, California. This position is designed for a Registered Nurse (RN) who will be responsible for care coordination and care transitions planning throughout the acute care patient experience. The RN Case Manager will work collaboratively with physicians, utilization managers, medical social workers, and bedside RNs to ensure the timely progression and transition of patients to the appropriate level of care, thereby preventing unnecessary admissions or readmissions. The role encompasses effective communication and resource coordination across the continuum of care, aiming to achieve optimal health outcomes, access to care, and appropriate utilization of resources while respecting patients' self-determination. The RN Case Manager will be involved in various tasks including reviewing initial physician admission care plans, gathering medical, psychosocial, and financial information from patients and families, and determining risk levels for readmission. The position requires the RN to functionally supervise and lead the healthcare team in developing comprehensive, cost-effective care coordination plans that meet the clinical needs of patients. Additionally, the RN will identify and refer quality and risk management concerns, oversee case management assistants, and ensure that patients meet level of care requirements. The role also involves monitoring length of stay, collaborating with financial counselors, and ensuring compliance with Medicare regulations. In the event of being assigned to the Emergency Department, the RN Case Manager will address complex clinical and social situations efficiently to avoid unnecessary admissions. The position requires active participation in patient rounds, effective documentation in electronic health records, and maintaining current knowledge of regulatory requirements. The RN will also be responsible for formulating transition plans, screening for readmissions, and proactively identifying barriers to care progression. Overall, this role is crucial in ensuring safe and effective transitions of care for patients in an acute care setting.
