About The Position
We are seeking a compassionate and experienced Travel Nurse - RN specializing in Case Management to join our team in Glendale, CA. This role involves inpatient case management with a strong focus on timely discharge planning. The successful candidate will work day shifts from 8:00 AM to 4:30 PM, with a commitment to every other weekend, totaling a 40-hour work week.
Requirements
- Previous inpatient RN case management experience in an acute hospital setting is required.
- Current Registered Nurse (RN) license in the state of California.
- Strong critical thinking and problem-solving abilities.
- Excellent communication and interpersonal skills.
- Ability to work collaboratively in a fast-paced environment.
Nice To Haves
- Case Management certification (CCM or ACM) preferred but not required.
Responsibilities
- Conduct comprehensive assessments of patients' needs and care requirements.
- Collaborate with interdisciplinary teams to develop and implement effective discharge plans.
- Monitor patient progress and adjust care plans as necessary to ensure optimal outcomes.
- Educate patients and families on discharge processes and post-discharge care.
- Maintain accurate documentation of assessments, interventions, and patient progress.
- Adhere to hospital policies and regulatory requirements regarding patient care.
