About The Position

Brookdale is seeking a dedicated and compassionate Travel LPN to support clinical care across two of our Sarasota communities— Brookdale Deer Creek Memory Care and Brookdale Sarasota Midtown . This unique position allows you to make a meaningful impact while working with residents in both assisted living and specialized memory care settings. As a Travel LPN, you will play a vital role in delivering person-centered care, ensuring resident safety, and collaborating with team members across both locations. This role is ideal for a nurse who thrives in dynamic environments, values flexibility, and is passionate about enhancing the quality of life for seniors. Key Responsibilities: Provide hands-on clinical care, medication administration, and health monitoring in accordance with state regulations and Brookdale policies. Travel between Deer Creek Memory Care and Sarasota Midtown based on daily staffing and resident care needs. Communicate effectively with residents, families, and interdisciplinary teams. Support dementia-specific programming and interventions, particularly at the memory care location. Requirements

  • Education as required to obtain state nursing license (LPN/LVN) and/or minimum of one year related experience and/or training; Additional education can be substituted for experience on a year for year basis.
  • LPN or LVN license.
  • Basic CPR/first aid training.
  • Fire safety training.
  • Valid Driver’s License
  • Depending on location of assignment, may be required to temporarily stay onsite at community or in alternate lodging provided by company.
  • Has a working knowledge of a skill or discipline that requires basic analytic ability.
  • Has an overall understanding of the work environment and process.
  • Has a working knowledge of the organization.
  • Familiarity with Microsoft Windows, Microsoft Office and e-mail (Microsoft Outlook or Exchange, preferred).
  • Basic typing skills are essential.
  • Ability to maintain resident confidentiality.

Responsibilities

  • Provides care to residents in accordance with physician orders, recognized standards of practice and established company policies and procedures.
  • Travels to various communities throughout district or state on a rotating schedule to cover temporary vacancies or to provide additional support.
  • Assists in maintaining a physical, social and psychological environment in the best interests of residents.
  • Ensures resident rights are maintained at all times.
  • Monitors residents within state licensure regulations; supervises and directs nursing assistants where allowed by state licensure regulations.
  • Assists in writing care plans based on resident needs and change of condition.
  • Obtains and administers medication and treatments as prescribed by physician.
  • Documents and reviews medication and treatment sheets for accuracy and compliance with physician orders, professional standards, federal and state regulations and company policy.
  • Monitors responses to care treatment plans.
  • Possesses knowledge of medication interactions and notifies Nursing Supervisor or designee of any issues.
  • Adheres to company systems, processes and procedures.
  • Counts controlled substances with off going nurse or designee; secures and corrects reconciliation of controlled substances during shift.
  • Ensures cleanliness and organization of medicine supply room and security of discontinued medicine in accordance with federal and state regulations and company policy.
  • Assists with discharge process relevant to medication and treatment orders.
  • Possesses knowledge of OSHA regulations, infection control standards, and emergency procedures.
  • Follows policies and procedures to ensure resident safety.
  • Documents all pertinent information regarding nursing care, care plans and observation of resident overall condition.
  • Attends training to maintain skill level and remain current in emerging healthcare trends.
  • Tracks mileage and other relevant travel expenses pertaining to assignment.
