About The Position
A Long-term care facility is seeking a dependable Licensed Practical Nurse (LPN) to provide high-quality nursing care to residents in a skilled nursing and rehabilitation setting. The ideal candidate is flexible, team-oriented, and comfortable floating within the facility. Facility & Unit Details Setting: Long-Term Care / Skilled Nursing Unit Size: 40-bed unit Floating: Required to all units within the same building Guaranteed Hours: 37.5 hours per week Patient Populations: Medical-Surgical Orthopedic Dementia Memory Care EMR System: PointClickCare
Requirements
- Active New York State LPN License
- BLS Certification (AHA preferred)
- Minimum 1+ year of Long-Term Care or skilled nursing experience
- Ability to float across units
- Strong communication and documentation skills
Responsibilities
- Administer medications and treatments per physician orders
- Monitor, document, and report changes in resident condition
- Perform wound care and basic nursing procedures within LPN scope
- Assist with activities of daily living (ADLs) as needed
- Collaborate with RNs, CNAs, and interdisciplinary team members
- Maintain accurate documentation in the electronic medical record
- Ensure resident safety, dignity, and quality of care
