About The Position
We are seeking a Licensed Master Social Worker (LMSW) to support case management and care coordination services in an acute care hospital setting. This role focuses on discharge planning, patient advocacy, and interdisciplinary collaboration across multiple hospital units. The position involves providing case management and discharge planning services for hospitalized patients, coordinating care transitions and post-discharge services, collaborating with physicians, nurses, and interdisciplinary teams, assessing patient needs and developing appropriate care plans, supporting patients in navigating healthcare and community resources, and documenting all interventions and care coordination activities in EMR. The role requires working across multiple hospital units as needed.
Requirements
- Active Washington LMSW license (required)
- Minimum 1 year of experience in acute care social work
- Experience in discharge planning and care coordination required
- BLS certification required
- Ability to work across multiple hospital units
Nice To Haves
- Experience in Med/Surg, ED, and length-of-stay case management
- Strong interdisciplinary communication skills
- Experience in large hospital systems
