About The Position

"Navitas Healthcare, LLC" is seeking a compassionate and experienced Home Health Registered Nurse (RN) for a travel assignment in South Dennis, Massachusetts . In this role, you will deliver high-quality, patient-centered care in a home health setting, providing one-on-one support to patients while managing an independent caseload. Requirements

• Active Registered Nurse (RN) license
• Minimum 1 year of RN experience OR 6 months of RN experience with a BSN
• Current CPR/BLS certification
• Experience with electronic documentation systems

Nice To Haves

• Home Health or Home Care experience preferred
• IV therapy experience preferred
• Bachelor of Science in Nursing (BSN) preferred

Responsibilities

• Patient Assessment Perform comprehensive admission and ongoing assessments, evaluating physiological, psychological, and environmental factors impacting patient care.
• Care Planning Develop, implement, and adjust individualized care plans in accordance with physician orders and patient needs.
• Case Management Manage assigned patient caseload while ensuring care aligns with established home health standards and benchmarks.
• Home Health Aide Supervision Provide documented supervision of Home Health Aide care plans every 14 days.
• Care Coordination Participate in interdisciplinary case conferences and communicate significant patient status changes to physicians and care team members.
• Clinical Documentation Maintain accurate, timely, and compliant documentation of skilled nursing services and patient responses using electronic health records.
• Discharge Planning Coordinate patient discharge planning and ensure proper documentation of patient outcomes and disposition.
