About The Position

Responsible for non-emergency secure transportation of individuals seeking and discharging from crisis services at our Acute Care facility. Transports individuals to the facility from various locations in Larimer County, as well as back into the community upon discharge from our facility. Picks-up client medications from pharmacies and fulfill other transport needs as needed.

Requirements

• High school diploma or equivalent required
• No experience required; experience preferred in working in behavioral health or other healthcare settings preferred
• Valid Colorado driver’s license and a clean driving record required

Responsibilities

• Transport clients to and from the acute care facility, working in close partnership with clinical/medical and care coordination teams.
• Fulfill requests to pick up discharge medications for clients preparing for discharge from the facility.
• Coordinate regularly with other facilities and community partners working to arrange transportation for clients.
• Participate in regular operations meetings/huddles to get assigned work each shift.
• Document trips in electronic medical record and other record keeping databases/software.
• Help to maintain vehicle fleet by ensuring routine maintenance is completed on vehicles.
• Provide compassionate and professional services to all clients.
