Make a Difference The Transfer Center RN Specialist is responsible for facilitating appropriate patient transfers into Community Health Network from referring physicians and facilities (hospitals, ED’s, clinics, etc.) Working in a Call Center environment, the Transfer Nurse Specialist obtains clinical information necessary to determine the appropriate consulting CHNw Specialists, medical necessity, level of care, and transport requirements. Applies clinical judgement to facilitate communication between the referring physician and collaborative CHNw receiving physicians per the anticipated level of care required. Effectively engages transport and clinical staff to ensure timely, seamless, & safe patient transfers to our receiving hospitals. Documents and monitors all activities associated with the transfer from initial physician referral to in patient admission and patient placement. The Patient Transfer Center is located at 13050 Parkside Drive in Fishers, IN.

Requirements

  • 3 years of acute care, cardiac telemetry, critical care/emergency nursing experience, case manager, charge nurse/house supervision, flight nursing or transfer center experience preferred.
  • Graduate of National League for Nursing (NLN) or Commission on Collegiate Nursing Education (CCNE) accredited school of Nursing, or 5 years related professional nursing experience; BSN preferred.
  • Licensed as a Registered Nurse (RN) by the Indiana Professional Licensing Agency (IPLA)
  • Certification Advanced Cardiac Life Support Certification Basic Life Support
  • Able to work with peers in a team situation.
  • Oral communication skills to discuss patient care needs.
  • Written communication skills as necessary for documentation.
  • Computer skills for documentation and work-related purposes.
  • Demonstrates a learning attitude toward solving problems.
