About The Position

Baystate Health, a nationally recognized leader in healthcare quality and safety, is looking for a part-time Social Worker to join our Transplant Services team in Springfield, MA. The transplant social worker participates in a program that offers the option of transplantation as a treatment modality for patients with End Stage Renal Disease. This role is an active member of a multidisciplinary care team that provides quality, safe, and compassionate care to our patient population. The social worker will report directly to the manager of Transplant Services and collaborate with the designated transplant multidisciplinary team in the initial assessment, care planning, intervention, reassessment, and discharge planning for kidney recipients and living donors.

Requirements

  • MSW degree from an accredited Social Work program.
  • Licensed LICSW for State of Massachusetts (required).
  • Experience in Renal Health is advantageous.
  • Spanish, Bilingual language skills are optional.

Responsibilities

  • Provides a comprehensive psychosocial assessment of patients.
  • Participates with team, patient, and family to develop a plan of care and provide counseling for long-range planning and discharge.
  • Accurately documents social work interventions in the patient record in accordance with agency policy.
  • Provides counseling, education, and support to patients and families related to identified medical conditions/stressors.
  • Develops and maintains a network with community referral resources.
  • Provides crisis intervention in consultation with supervisor.
  • Supports and educates staff regarding psychosocial and spiritual aspects of patient care.
  • Conducts interviews with patients and their families/support persons to assess and identify issues.
  • Coordinates input from all healthcare professionals, patient, and family members to formulate a documented plan of care.
  • Provides clinical consultation as an active member of the interdisciplinary care team regarding psychosocial issues.
  • Plans and coordinates patient care conferences including follow-up interventions.
  • Enters data and information into the electronic medical record and maintains accurate clinical records.
  • Develops and maintains working relationships with community agencies.
  • May supervise social work interns and/or LCSW social workers obtaining required supervision hours.
