About The Position

The Transplant Education Nurse Coordinator at the University of Iowa's Organ Transplant Center is responsible for educating transplant patients and their families about the transplant process, post-transplant care, and medication management. This role involves collaboration with various healthcare professionals to ensure safe patient discharges and improve post-transplant outcomes. The coordinator will also participate in quality improvement initiatives and provide education to other healthcare staff and potential transplant candidates.

Requirements

  • Baccalaureate Degree in Nursing is required.
  • 1-3 years of nursing experience required.
  • Experience with electronic health records is required.
  • Current Iowa Nursing License.

Responsibilities

  • Improve post-transplant outcomes and patient compliance.
  • Collaborate with the inpatient team to facilitate timely and safe discharges for transplant patients.
  • Document discharge using transplant note templates and electronic medical record tools.
  • Attend inpatient and outpatient team rounds as scheduled.
  • Participate in the discharge process for transplant recipients and donors.
  • Observe for patterns of variation from the discharge plan and facilitate corrective actions.
  • Prepare patients and families for post-transplant issues, including medication regimens and self-monitoring.
  • Assess patient retention of complex instructions and modify teaching methods as needed.
  • Provide education to potential transplant candidates and dialysis unit staff.
  • Lead staff education efforts and document educational activities with patients.
  • Collaborate with post-transplant nurse coordinators for patient discharge coverage.
  • Educate and collaborate with Transplant Navigators for post-transplant supply bags.
  • Participate in systematic reviews of protocols related to the transplant population.
  • Provide on-call coverage and coordinate activities prior to transplant.
  • Facilitate communication between physicians, organ procurement organizations, and families during the transplantation process.
