About The Position
The Transplant Coordinator RN 2 is responsible for coordination of the evaluation and preparation of patients for organ transplantation and long-term management of patients after transplant. The Transplant Coordinator is responsible for coordinating care across multiple areas in order to provide continuity of patient care across settings. This position is also responsible for performing initial assessment of transplant evaluated patients and providing follow-up post-transplant care, which could be performed in hospital or outpatient clinics. The individual who holds this position exemplifies the Rush mission, vision, and values and acts in accordance with Rush policies and procedures.
