As a Transitions of Care (TOC) Behavioral Health Social Worker, you will focus on Transitions of Care (TOC) support for Devoted members. You will provide short-term interventions to members in the process of being connected to community-based behavioral health services, either because of an emerging need or following a hospitalization. The goal is to prevent avoidable emergency room/inpatient care and/or prevent readmission to the hospital. The TOC team is dedicated to providing culturally competent care with a focus on equity and quality. We pay close attention to our team culture, treating each other like family so we have the requisite resiliency to care for our members like family.
