About The Position

The Transitional Specialist is responsible for providing skills training to young adults with disabilities. The Transitional Specialist is further responsible for ensuring that support services are consumer directed, enhance individual competencies, promote meaningful relationships and maximize individual learning potentials. Exposing students age 14-22 to skills needed to move them forward in developing a plan for their next steps in life. Responsible for incorporating the values and principles of the Agency's mission. Training, supervising and working with individuals with disabilities in a variety of community, vocational, and educational facilities. Implement discovery based techniques that look to examine where students are at their best in their lives and translate the needs to move the student forward. Provide support to the student on the job (internships) by understanding and maintaining the necessary tasks and requirements. Compose clear and concise objective/subjective progress notes. Attend IEP meetings, risk management meetings and close out meetings. Adhere to regulations regarding DDPC, OSHA, and DOL. Efficient and proper use and maintenance of assigned equipment and supplies. Implementing components of each program service plan 10. Safety of the al students under your supervision.

Requirements

  • High School Diploma / Some college courses
  • Two - three year experience working with young adults with disabilities.
  • Driver's License required and safe driving record.

  • Second language skills including American Sign Language preferred
