Provide supervision, support and training to people with differing abilities served by Lumibility through group instruction. Implement activities that are person-centered and promote positive outcomes based on individual and group focused goals. Provide the environment and supports necessary to promote appropriate social skills and positive behaviors that will enhance skills, enrich lives, and foster community presence & participation. Pay Rate: $19.50/hour Hours: Full Time 40 Hours/week Ensure the health, safety and well-being of people served by Lumibility in accordance with the individual service plan and in compliance with agency policies and procedures. Provide supervision, guidance and instruction to individuals served by Lumibility at community-based assessment sites and during structured curriculum based instructional opportunities who are served under the Transitional Services program. Support services can include, but are not necessarily limited to: direct supervision, behavioral interventions, attending team meetings, skill development and providing effective communication with supported individuals at community assessment sites and businesses. Provide transportation for assigned service individuals in accordance with Lumibility safe-driving policies and practices. Have knowledge of assigned community assessment sites to provide effective supervision and guidance Maintain medical, social and programmatic files by recording each person’s progress and documenting other required information. This includes, but is not limited to: service goals, attendance records, behavioral data, progress notes, and other documentation as required or needed. Develop and maintain positive relationships with service participants, families, co-workers and other community representatives. Other duties, assignments and work locations as assigned
