About The Position

The Transitional Housing/Rapid Re-Housing (TH/RRH) Case Manager is an experienced social service provider who assists families who are actively or were recently fleeing domestic violence. The families may live at the SAMM Transitional Living & Learning Center (TLLC) and will soon move into permanent housing. The TH/RRH Case Manager works collaboratively with families to develop goals and objectives to help them obtain and remain in permanent housing, increase their skills and/or income, learn domestic violence prevention skills, and help them achieve greater self-determination. The Case Manager provides information and referrals to community resources and other organizations relevant to the resident’s service plan and goals.

Requirements

  • Bachelor’s Degree from a 4-year college or university, preferably in social work, or in another behavioral science
  • At least 2 years’ case management experience, preferably with clients experiencing homelessness, or with other highly vulnerable populations
  • Strong written and verbal communication skills
  • Ability to maintain current and complete case documentation
  • Valid Texas Driver’s License, a reliable motor vehicle, and liability insurance
  • High level of energy, creativity and flexibility

Nice To Haves

  • Experience working with survivors of domestic violence preferred
  • Bilingual (English/Spanish) is preferred

Responsibilities

  • Provide case management services that facilitate client self-sufficiency, housing goals, and life-skills education
  • Ability to build positive and trusting relationships with diverse populations, particularly participants who have experienced trauma, domestic violence, homelessness, physical and/or mental health disabilities, and substance use challenges
  • Attend training for, and utilize evidence-based practices in service delivery, such as Trauma Informed Care, Motivational Interviewing, Harm Reduction, Housing First, and Person-Centered Planning
  • Attend Home Visit Safety Training, and utilize the safety practices in daily work
  • Work with clients who are living in the TLLC (TH), as they prepare to live in a RRH rental unit in the community
  • Continue working with clients who have moved into RRH rental units in the community by conducting home-visits
  • Offer guidance to clients through their housing search, lease application, and moving process
  • Conduct BioPsychoSocial Assessments and collaboratively assist residents in the development of Individual Service Plans; to include goals for housing stability, increased skills and/or income, and personal self-enrichment
  • Actively involve residents in the design and delivery of supportive services
  • Refer participants to necessary services and coordinate with other providers to help participants to access services and achieve goals set in the Individual Service Plan
  • Provide education on domestic violence prevention and complete Safety Plans with survivors of domestic violence
  • Maintain on-going case load of 22-26 families participating in the program by meeting with each participant on a weekly, biweekly or monthly basis (determined collaboratively with the Program Manager)
  • Encourage Residents to attend psychoeducational groups that focus on the life-skills necessary for healthy relationships, long-term financial empowerment, overall health, and housing stability
  • Identify barriers, problem-solve, and work with clients to find solutions to those barriers pertaining to their goal achievement
  • Work in collaboration with other professionals, agency staff, and volunteers, to include property managers and other housing service providers across the community
  • Attend weekly Staffing Meetings to review resident progress, problem-solve, and plan services
  • Maintain necessary documentation, reports, and statistics on internal databases as well as in the Homeless Management Information System (HMIS) database
  • Must be available to assist with the facilitation of some occasional evening and weekend events at the TLLC facility
  • Other duties as assigned
