About The Position

The Transitional Housing Homeless Outreach Worker serves as the initial point of contact for homeless youth aged 16 to 24, providing essential support and resources to help them navigate their crises. This role emphasizes a nonjudgmental approach to outreach, case management, and counseling, ensuring that youth receive the necessary assistance to improve their situations.

Requirements

• Experience working with homeless youth or in social services.
• Strong communication and interpersonal skills.
• Ability to provide case management and counseling effectively.
• Knowledge of community resources available for homeless youth.

Responsibilities

• Connect resources to homeless youth and provide a nonjudgmental approach to crisis resolution.
• Provide case management and counseling to youth in crisis on the streets.
• Make decisions regarding qualifications for Catholic Charities services for the homeless youth housing program and onsite transitional house.
• Respond to referrals coming to the program and make necessary referrals to other programs.
• Perform screening/intake with new referrals and outreach youth, explaining services and completing required paperwork.
• Maintain professional relationships with community resources.
