About The Position

BHcare is seeking a compassionate and dedicated Transitional Counselor to join our Transitional Supervised Housing Program. In this role, you’ll provide individualized, goal-oriented support that helps residents build independence, enhance life skills, and integrate successfully into the community. Schedule: • Tuesday: 3pm – 11pm • Saturday & Sunday: 3pm – 11pm What You’ll Do: • Assist in the daily operations of the program while ensuring a safe and supportive environment. • Provide individualized recovery planning and ongoing support tailored to residents’ strengths and needs. • Teach and coach independent living skills such as personal care, cooking, cleaning, money management, and safety. • Facilitate rehabilitation groups and provide interventions aligned with recovery plans. • Offer crisis intervention when needed and coordinate care with clinical teams, emergency personnel, and community resources. • Supervise self-medication protocols and support residents in developing healthy habits. • Maintain accurate documentation, progress notes, and data entry. • Collaborate with treatment teams, families, and community partners to connect residents with vital supports. • Stay awake and alert during all shifts to ensure resident safety and program operations. Requirements

  • Experience working with individuals with mental health and/or co-occurring disorders.
  • Strong communication, empathy, and problem-solving skills.
  • Ability to teach life skills and foster independence with patience and respect.
  • Flexibility to respond to crises and adapt to residents’ evolving needs.
  • Willingness to use personal vehicle for company business or to transport clients.
  • Two years of college education (human services or behavioral science preferred) or High School Diploma or Equivalent and 2-4 years direct care direct care work experience.
  • Must be proficient in Microsoft 365.
  • Must be able to navigate through electronic health records system (EHR).
  • Must have and maintain a valid driver’s license.
  • Must maintain an acceptable driving record and be insurable at a reasonable rate under the Organization’s auto insurance plan.
  • Ability to obtain and maintain BLS certification
