About The Position

The Transitional Care Specialist position at the State of Indiana involves working with residents at the Neurodiagnostic Institute, focusing on implementing treatment plans and providing support to individuals with psychiatric illnesses. Requirements

  • High School Diploma or HSE diploma
  • 1+ years of experience in a similar role
  • Certified or willing to become certified in CPR/AED
  • Specialized knowledge of psychiatric nursing care practices
  • Ability to write reports and keep records accurately
  • Ability to respond effectively in emergency situations
  • Knowledge of behavior modification and self-defense techniques
  • Working knowledge of resident's behavior
  • Ability to perform housekeeping duties and daily living activities
  • Knowledge of therapeutic rehabilitation programs
  • Knowledge of needs of mentally ill client groups
  • Practical knowledge of teaching techniques
  • Ability to operate routine medical equipment
  • Ability to complete required documentation timely
  • Ability to legally operate a motor vehicle in Indiana
  • Ability to maintain and distribute client trust accounts
  • Ability to recognize behaviors in clients and take appropriate action
  • Working knowledge of existing mental health provider systems

Responsibilities

  • Implement processing and education groups
  • Documentation for each group
  • Participate in monthly treatment team documentation
  • Ensure security of residents and maintain orderliness
  • Escort residents to and from various activity and therapy areas
  • Assist in the implementation of resident's treatment plan
  • Monitor patients during meals, classroom time, and general areas
  • Observe, report, and record the condition and behavior of residents
  • Demonstrate therapeutic interventions using de-escalation and approved techniques
  • Assist residents in cleaning and caring for their living areas
  • Secure doors, search visitors, and operate radio system
  • Conduct searches of patient's rooms for contraband
  • Provide motivational techniques and counsel to clients
  • Conduct ongoing assessments of clients' training needs
  • Advise clients regarding appropriate social behavior
  • Provide a safe environment conducive to learning
  • Coordinate client activities under professional direction
  • Develop clients' community living skills through activities
  • Maintain clients' money and distribute it appropriately
  • Assist in carrying out occupational therapy/recreational therapy programs
  • Perform routine clerical functions
  • Obtain drugs from the pharmacy and administer under RN's direction
  • Assist in managing aggressive behavior of clients
  • Perform basic first aid as necessary
  • Attend worksite as an essential function.
