About The Position

The Transitional Care Nurse (RN) at CarDon and Associates plays a critical role in ensuring seamless resident transitions from hospital to home or other care settings. This position focuses on providing comprehensive nursing care that supports resident recovery, reduces readmission rates, and promotes overall health and well-being. The Registered Nurse (RN) will coordinate with multidisciplinary teams to develop and implement individualized care plans tailored to each resident's unique needs. By closely monitoring resident progress and educating residents and families, the Transitional Care Nurse (RN) helps empower them to manage their health effectively. Ultimately, this role is essential in bridging gaps in care and improving resident outcomes during vulnerable transition periods.

Requirements

  • Current and valid Registered Nurse (RN) license in the state of Indiana.
  • Proven experience in resident care, particularly in transitional or post-acute settings.
  • Certification in CPR and First Aid.
  • Strong communication skills, including proficiency in answering phones professionally.
  • Basic knowledge or experience with dialysis procedures.

Nice To Haves

  • Previous work experience in a home health or community care environment.
  • Additional certifications related to chronic disease management or case management.
  • Familiarity with electronic health records (EHR) systems.
  • Advanced training in resident education and counseling.

Responsibilities

  • Conduct thorough resident assessments to evaluate health status and care needs during transitions.
  • Develop, implement, and update individualized care plans in collaboration with healthcare providers and family members.
  • Provide direct resident care including medication administration, and wound care as needed.
  • Educate residents and caregivers on disease management, medication adherence, and lifestyle modifications to promote recovery.
  • Communicate effectively with residents, families, and interdisciplinary teams, including answering phones and coordinating follow-up appointments.
