About The Position

A Transitional Care Management Nurse will function as an intermediary between the medical teams and the patients. In this capacity, the role is designed to accomplish the following items for all three campus locations within SRHS: first line support of appointment dates and times given to the patients before discharge with a goal of 95%, completion of part 1 of the TCM phone call process with a goal of 90%, and provide applicable E&M codes to providers based on insurance coverage. Acute awareness is needed to prevent unnecessary readmissions, improve HCAHPS, drive positive discharge outcomes and reduce LOS, and review ER utilization using proactive approaches to any barriers. Requirements

  • Must be a graduate of an accredited School of Nursing.
  • Currently licensed to practice in the State of Mississippi, License must be free of any encumbrances
  • Must be currently registered with the Mississippi State Board of Nurse Examiners
  • Must have (2) or more years direct patient care experience or clinic experience.
  • Must demonstrate keen mental faculties/assessment and decision making abilities.
  • Must demonstrate superior communication/speaking/enunciation skills to receive and give information in person and by telephone.
  • Must demonstrate strong written and verbal communication skills.
  • Must possess emotional stability conducive to dealing with high stress levels.
  • Must demonstrate ability to work under pressure and meet deadlines.
  • Attention to detail and the ability to multi-task in complex situations is required.
  • Must have the ability to maintain collaborative and respectable working relationships throughout SRHS and other organizations
  • Must possess superior customer service skills and professional etiquette.
  • Must possess proficient knowledge and ability to use a computer (must be keyboard proficient) and other office technology (i.e., telephone, fax, etc.), MS Outlook and Word.
