About The Position
Touchstone Communities is seeking a dynamic Transitional Care Liaison to drive census growth through strategic sales and referral development in the Killeen/Harker Heights region. As a Liaison, you'll represent Touchstone's skilled nursing communities by promoting our outstanding clinical outcomes and patient-centered care. Using a value-based sales approach, you'll build strong relationships with hospital discharge planners, physicians, and other healthcare professionals to ensure seamless transitions into our communities.
