This role conducts assessments, delivers education and coaching on health conditions, medications, and self-management, and connects participants to appropriate community and agency resources.

Requirements

  • Applicants must hold a valid, active, and unrestricted RN, LPN, or LSW issued by the state of Ohio.
  • Hold a minimum of one year of clinical experience post-licensure.

Responsibilities

  • Orient and elucidate program details to program participants, including medication information, care scope, transition event scope, coaching process, program expectations, and agency resources.
  • Conduct visits with program participants in their homes or assisted living facilities, either in-person or telephonically.
  • Administer and document all prescreening assessments, chart histories, and initial intake evaluations for each program participant.
  • Offer coaching to participants regarding their personal goals, conditions, medication adherence, medical appointment follow-ups, and self-advocacy.
  • Educate and provide support to program participants and their caregivers about the resources available through the Agency.
  • Act as an advocate for participants and/or caregivers/families, assisting in securing appropriate services in accordance with the participant's insurance policy and the agency's available resources.
  • Apply clinical expertise and critical thinking to execute program activities following education, experience, and relevant professional practice standards.
  • Report instances of abuse, neglect, or exploitation of older adults as mandated by Ohio law and program guidelines.
  • Adhere to the professional code of ethics established by the Ohio Board of Nursing, Ohio Board of Counselor, Social Work, Marriage & Family, and WRAAA.
  • Demonstrate consistent effective customer service behaviors and practices when interacting with both internal and external stakeholders to achieve departmental goals, program metrics, and agency mission.
  • Participate in training, meetings, in-services, and continuing education opportunities offered by the agency, managed care partners, or as identified by the employee. Also, respond to requests from supervisors, managers, or directors to support departmental goals.
  • Possess the ability and willingness to travel to community and participant locations within all county service areas (Cuyahoga, Lorain, Geauga, Lake, and Medina counties).
  • Collaborate with external agencies, internal departments, and partner AAAs to ensure seamless care continuity for program participants.
  • Fulfill other assigned duties as required by Management to meet agency and departmental objectives.
  • This role primarily involves tasks related to the effective operation of the Agency, necessitating the exercise of discretion and independent judgment in matters of significance.
  • Support and participate in additional programs as needed.
