Transitional Care Coordinator I or II, Full Time, Day Shift
About The Position
The Transitional Care Coordinator II is knowledgeable about disease, treatment, available community and governmental resources, psychosocial implications for individuals and families as it pertains to their age and stage of development; provides appropriate interventions and discharge planning services to the patients and families; and is pragmatic in approaching end of life and ethical issues. Communication is key to the role and various modes of communication are utilized. Performs all other duties as assigned by Transitional Care Lead and Case Management Director.
