Requirements

  • Minimum of Masters' Degree in Education, Social Work, or Counseling
  • DESE Transition Specialist Endorsement
  • Experience working with transition-aged students with disabilities and their families in public school, college, and employment settings, and throughout the community
  • Familiarity with MA state agencies and the services they provide for individual's post-high school
  • Familiarity with the Brockton area/community
  • Flexible schedule to accommodate trainings/workshops/activities and coverage/student schedules as needed

Responsibilities

  • Work collaboratively as member of Transition Team on behalf of BPS's SWDs ages 13 + across the District
  • Provide professional development training/technical assistance on Transition topics to BPS staff in various roles (teachers, paras/MTAs, administrators, related service providers, counselors, etc.)
  • Consult/collaborate with BPS staff to provide support and resources as needed, enabling them to complete transition assessments with students/families
  • Conduct select transition assessments as appointed/assigned by administrators
  • Attend Team Meetings, as well as various meetings in the community (such as statewide Transition Specialist meetings) as required/needed
  • Provide trainings to parents via SEPAC on Transition topics as needed/requested
  • Conduct Resource Fairs highlighting Transition and Related Community Services
  • Serve as resource person/consultant for parents, students, and BPS staff on Transition-related topics (such as state and community agencies that can assist SWDs, application processes/requirements, etc.)
  • Collaborate with students, families, BPS staff, and agencies for individual student case coordination
  • Develop, maintain, and update spreadsheets and other systems for tracking exiter information on Transition-aged SWDs
  • Serve as BPS contact person/liaison with state and other agencies serving SWDs (DDS, MRC, BTP, SSA, BAT, etc.)
  • Address disability documentation requests, including collaboration with other BPS staff as needed
  • Oversee 688 referral system, including supporting BPS staff in various roles as questions/needs arise, and tracking student referrals
  • Coordinate select transportation projects, including providing BAT one-ride tickets to BPS staff/students as needed/requested (for regular internship sites, student employment, and community outings), and First Student transportation arrangements (for BSU weekly programming)
  • Serve as BPS contact person/liaison to Bridgewater State University Serve as BPS contact person/liaison to other local universities, colleges and community colleges when applicable for any community, college or transitions programming or initiatives
  • (BSU) for the EXCEL Partnership, Transitions at Bridgewater (T@B), and Self-Advocacy Leadership Series (SALS) Programs
  • Work directly with students on transition topics (such as Transition Tips, Senior Transition, resumes/job applications, self-referrals to MRC, disability disclosure in the workplace or post-secondary education, self-directed IEPs, self-advocacy skill development, etc.) as needed/schedule permits
  • Attend Special Education Department Meetings as needed
  • Other duties as needed/assigned by Assistant Director of Special Education and/or the Associate Director of Exceptional Learning
