The Transitions of Care (TOC) Supervisor provides clinical and operational leadership for hospital-based TOC services across the health system. This role ensures standardized, high-quality transitional care practices across multiple hospitals while supporting performance improvement, interdisciplinary collaboration, and alignment with system-wide goals related to quality, safety, and value-based care.

Requirements

• Currently licensed as a Registered Nurse/Advanced Practice Registered nurse in the state of Georgia
• National Board Certification (e.g., FNP-BC, AGACNP-BC).
• Master’s degree in nursing (MSN) or Doctorate in Nursing Practice (DNP) from an accredited school of nursing.
• Credentialed by Northside Hospital Medical Staff office
• Three (3) years of inpatient Transition of Care experience

• Ability to assist staff in times of crisis or emergency.
