Under the general supervision of the Systems Director of Pharmacy and the direction of the Retail Pharmacy Manager/Outpatient Pharmacist In Charge, the Transitions of Care (TOC) Pharmacist is responsible for the coordination and direction of the Transitions of Care Pharmacy Technicians based in the Outpatient pharmacy. The Transitions of Care Pharmacist alongside technicians provide services at admission such as, but not limited to, medication history, and identifying high risk patient populations. Services provided during the discharge process include, but not limited to, compliance with guideline driven medication therapy, prior authorizations, discharge counseling, Meds to Beds services. The Transitions of Care Pharmacist will work closely with providers, technicians, nursing staff, case management staff and other pharmacists. The Transitions of Care Pharmacist will also assist retail pharmacy staff with dispensing and verification of discharge prescriptions.
