Requirements

  • One year of full-time behavioral health work experience
  • Bachelor's degree in Behavioral Health, Social Services, or a related field
  • Must be able to pass a Maricopa County Sheriff's Office (MCSO) human resource fingerprint background check and qualify for a Jail access badge; previous legal history does not eliminate you from this process

Nice To Haves

  • Experience as a Peer Specialist, Peer Support, or Peer Navigator
  • Licensed Associate Substance Abuse Counselor (LASAC)
  • Completion of training in providing comprehensive peer support such as the Arizona Peer Support Employment Training Program, Certified Peer Support Specialist Training, or the Forensic Peer & Family Support
  • Experience in crisis intervention and suicide risk prevention/education
  • Experience in organized task management, efficient data gathering, and coordination of large amounts of information to prioritize linkage to care interventions
  • Experience with triaging internal and external referrals for connection to services
  • Working knowledge of corrections, behavioral health community treatment services, and mental health/substance use terminology
  • Bilingual in Spanish and English

Responsibilities

  • Provides peer support, case management, and behavioral health navigation services for justice-involved individuals to assist them in successfully transitioning from incarceration back into the community
  • Connects individuals to harm reduction services
  • Develops and coordinates community re-entry plans that address risk factors associated with recidivism to the criminal justice system
  • Maintains knowledge of community resources and contacts to support patient's effective transition back to the community
  • Provides education on overdose prevention and other preventative measures to address risk factors, e.g., Suicide prevention
  • Obtains treatment records and coordinates care with community providers and others relevant to healthcare and community re-entry plan
  • Works collaboratively with program evaluator to ensure timely data collection and reporting are completed in an accurate and timely manner (this is ASU)
  • Uses an electronic health record
  • Maintains HIPAA patient confidentiality in the management of information
  • Coordinates with the multidisciplinary team in the management of patient care and departmental objectives, including participation in mental health staff and department meetings and initiatives
  • Adheres to all applicable professional standards and requirements, including modeling the code of ethics associated with licensure and certification and meeting regulatory, licensure, accreditation, health assessment, and CPR requirements
