About The Position

Comprehensive Healthcare provides innovative behavioral health and integrated healthcare services to clients of all ages. The Transition Coordinator has the important role of ensuring that each client leaving the assigned evaluation & treatment facility is prepared with a collaborative, comprehensive discharge plan to help create a smooth and safe transition back into the community. Starting at intake, the Transition Coordinator works closely with each client to assess their prior experiences, financial resources, physical health needs, and available support systems to determine the best possible outcome upon discharge from our E&T into outpatient treatment. The Transition Coordinator facilitates communication between the client and the treatment team to ensure that their discharge plan emphasizes the client’s goals & autonomy while also creating a safe transition for the client’s continued stability. Knowledge of recovery principles, a commitment to trauma-informed care, exceptional communication skills, and a desire to support individuals during a time of crisis are essential in this role. This position will be full-time Monday through Friday 8am to 5pm.

Requirements

  • Bachelor's degree in social work, psychology, or related field
  • Must be eligible for registration as Agency Affiliated Counselor
  • Driver's license and vehicle liability insurance
  • First Aid/CPR Certification

Responsibilities

  • coordinating communication between the client, their family, the treatment team, outpatient clinicians, and other services providers as necessary
  • participating in team meetings to gather and share relevant information about each client and their treatment progress
  • developing a comprehensive discharge plan for each client and facilitating the communication process between inpatient & outpatient programs to ensure the safe and efficient transition of each client
  • creating a safe, therapeutic, trauma-informed environment for clients
  • monitoring clients’ symptoms, response to medications, and risk to self or others and taking appropriate intervention steps when indicated
  • completing all documentation in a timely, client-centered manner
