The Transition Coordinator is responsible for developing and implementing strategies to support youth with disabilities in their career path through the PRE-ETS model. This role involves leading a team to provide student-directed support services that enhance individual competencies and promote meaningful relationships, ultimately maximizing learning potential for students aged 14-22.

Develop and implement effective strategies based on the PRE-ETS model.
Lead the team in providing services to youth with disabilities.
Ensure support services are student-directed and enhance individual competencies.
Promote meaningful relationships among students and community partners.
Maximize individual learning potential for students aged 14-22.
Expose students to skills needed for their next steps in life.
Conduct meetings and present material to small and large groups.
Network and outreach to community partners.

High school diploma or GED required.
Minimum of 2 years working with high school age youth.
Strong oral and written communication skills.
Ability to respond to inquiries and complaints professionally via email, text, or telephone.
Drivers license with a safe driving record and reliable vehicle.
Computer literate with knowledge of Microsoft Office Suite, Salesforce, Jotform, and Padlet.

Bachelor's degree in special education, psychology, or vocational rehabilitation preferred but can be waived for experience.
Lived experience in the disability community.
Second language skills including ASL preferred but not required.

Full-time position with competitive pay of $22 - $24 an hour.
