About The Position

We are currently seeking a Transition Coordinator Supervisor to join our HOME Choice program. This is a meaningful opportunity for professionals who are passionate about helping others live healthier, more independent lives. As a Transition Coordinator Supervisor, you’ll oversee a team of Transition Coordinators who arrange for services prior to and following an individual’s relocation from an institutional setting to the community, facilitating a safe, orderly and sustainable discharge. The Supervisor will assist the Clinical Manager in guiding, supporting and aiding in the evaluation of the program focusing on program requirement compliance and task assignment compliance

Requirements

  • An undergraduate degree in public health, economics, sociology, psychology, business administration, social work or nursing or other related discipline.
  • 36 months of experience providing services under Medicaid.
  • Effective oral and written communication skills with attention-to-detail.
  • Effective interpersonal skills with respect for others, including individuals served, co- workers and other professionals within a diverse work environment.
  • Proven ability to exercise independent judgment and decision-making, critical thinking and problem- solving skills.
  • Proficient computer skills and familiarity with suite of Microsoft Office programs.

Nice To Haves

  • Experience involving direct contact with individuals with disabilities preferred with extensive knowledge of community resources.
  • Experience in data analysis and/or data trending and/or accounting or financial analysis desirable, as well as familiarity with quality improvement processes.

Responsibilities

  • Adhere to the CareStar Rule in performance of job responsibilities
  • Understand and comply with CareStar Policies and Procedures.
  • Maintain confidentiality as related to patient information. Any disclosures of confidential information made unlawfully outside the proper course of duty will be treated as a serious disciplinary offense.
  • Follow the Acceptable Use Policy while using any information systems owned or controlled by CareStar, Inc.
  • Directly supervise program staff, including hiring, educating or coaching, disciplinary actions, completing performance evaluations, and setting individual development goals and objectives.
  • Assists the Clinical Manager in audit/review of communication notes, documentation and incident reports, implementing quality improvement activities as necessary.
  • Perform duties and responsibilities of program staff (e.g. Transitions Coordinators) and acts as backup during department absences as business needs require.
  • Collaborates with Case Managers and other HOME Choice team members to improve communications and achieve improved efficiencies.
