About The Position

The Transition Coordinator RN's primary responsibility is to deliver the most remarkable patient experience, in every dimension, every time, and understand how he or she contributes to the health system's vision of achieving that commitment to patients and families. The Transition Coordinator RN is an experienced nurse who is able to provide 1:1 instruction and education to patients/families who will and have experienced a surgical procedure in their area of focus (e.g., cardiothoracic, orthopedic). In addition, this position will serve as patient advocate, discharge nurse, and provide follow‑up calls to assess and assist the patient's transition home. The Transition Coordinator RN will make appropriate referrals for follow‑up care and rehabilitation (e.g., cardiac rehab, physical therapy) and provide ongoing continuity between the patient, their surgeon, and their primary care physician until the transition of care back to the PCP has been completed . Requirements

  • Bachelor's Degree in Nursing
  • Washington Registered Nurse License upon hire
  • Acute care experience in the specialty area assigned (ex Cardiovascular CC, Orthopedic surgery, post-op care)

Nice To Haves

  • Master's Degree in Nursing
  • Experience as appropriate to specialty (ex Cardiac Rehabilitation, Out-patient cardiac nursing care, out-patient orthopedics, PT)
  • Case Management/Navigator experience

Responsibilities
