Hands of Heartland is looking for a compassionate and caring individual to work with the people we support as a Transition Coach or Para in Grand Island, NE. What You’ll Do: Under the guidance of a certified Special Education teacher, you will work with one or more school-age students with an IEP to provide targeted academic skills, daily living skills and social/emotional support to reduce or eliminate barriers to independent adult living. Students may need assistance with self-care and/or mobility. Coaches will work with students on their IEP goals and do daily data recording on these goals. Coaches will also work with students on daily living skills in the classroom and out in the community. You will assist students in transitioning from school to adult living by providing real life experiences and helping them to be as independent as possible.
