About The Position

As a Transitional Care Navigator, you'll serve as a compassionate and knowledgeable resource for individuals, families, physicians, and hospital referral sources seeking skilled or long-term care within the WesleyLife network. Guided by the One Wesley philosophy, the Navigator ensures a seamless and person-centered transition by collaborating closely with referral sources and the intake team, offering solutions across all communities to meet individual needs with excellence and efficiency.

Requirements

  • Bachelor's degree preferred, with at least two years of experience in senior living, long-term care, or healthcare.
  • OR
  • A minimum of two years of healthcare experience in a relevant setting (e.g., senior living, long-term care, hospital, home health, hospice) without a degree.
  • Excellent observation, verbal and written communication skills, problem solving skills, critical thinking skills, basic math skills, and computer literacy to meet competency standards.
  • Must be organized and have the skillset to manage multiple functions under stressful conditions and changing priorities.
  • Demonstrates the ability to coordinate short-term rehab admissions with accuracy, urgency, and empathy.
  • Maintain a customer-first mindset, treating referral sources and families as valued partners in the transition process.

Responsibilities

  • Triage resident referrals and all resident calls regarding Wesleylife services and assist with directing residents to services they may need which includes our Hospice and Home Community Based Services.
  • Answer incoming calls promptly and courteously, demonstrating WesleyLife's commitment to hospitality and excellence while gathering essential information and understanding the customer's needs.
  • Retrieve information from the referral source platform and send it to the community of choice for the clinical nurse review process at the community level. Once potential admission is approved or declined, follow up with referral sources immediately.
  • Collaborate closely with the intake team and hospital partners to ensure timely and appropriate bed placement, prioritizing the customer experience and continuity of care.
  • Maintain a system-wide perspective, ensuring every referral is supported-even if it means coordinating across multiple communities.
  • If the community of choice is full, offer another Wesleylife community and collaborate with intake team and hospital partner on bed placement.
  • Serve as a primary liaison to hospital discharge planners, ensuring they receive exceptional customer service throughout the referral and transition process.
