Requirements

  • Bachelor's Degree Nursing required
  • Registered Nurse [RN - State License] - Generic - HR Only required
  • Basic Life Support [BLS Certification] - Data Conversion - Various Issuers required
  • Clinical experience in acute care or critical care settings 2-3 years required
  • Patient transfer experience 1-2 years required
  • Strong communication and interpersonal skills.
  • Ability to work collaboratively with diverse healthcare teams.
  • Knowledge of regulatory requirements and standards related to patient transfers.
  • Critical thinking skills and ability to make quick and accurate decisions in emergency situations.
  • Commitment to patient-centered care and excellent customer service.

Responsibilities

  • Coordinate the transfer of patients within the hospital or between healthcare facilities.
  • Serve as the primary point of contact for communication with sending and receiving facilities.
  • Conduct patient assessments to determine the clinical status and medical needs before transfer.
  • Ensure that necessary medical documentation and patient records accompany the transfer.
  • Complete and maintain accurate documentation related to patient transfers, including medical records, transfer summaries, and communication logs.
  • Coordinate the allocation of necessary resources for patient transfers, including appropriate medical personnel, equipment, and transportation.
  • Provide support and information to patients and their families regarding the transfer process.
  • Address concerns and ensure that patients and families are informed and comfortable with the transfer plan.
