About The Position

The Transfer Center RN Coordinator PRN at TriStar Skyline Medical Center is responsible for facilitating patient transfers by collecting clinical information and matching patients' needs with appropriate facilities. This role involves coordinating the transfer of patients from various referral sources, ensuring timely and efficient patient throughput, and maintaining accurate data in relevant systems. The position requires strong communication and problem-solving skills to interact effectively with healthcare professionals and manage multiple tasks simultaneously.

Requirements

  • Graduate of an accredited school of Nursing; BSN preferred.
  • Current licensure as a Registered Nurse in the State of Tennessee required.
  • Three years of related clinical experience in an acute care setting (ED or Critical Care preferred).
  • Ability to work on-site at the Central Region Patient Logistics Center in Madison, TN required.
  • Availability for 12-hour day shifts, rotating weekends, and holiday shifts required.

Responsibilities

  • Collect clinical information related to transfer requests.
  • Facilitate patient transfers by matching clinical needs to facility capabilities.
  • Coordinate the transfer of emergent, urgent, and elective patients to final destination facilities.
  • Ensure timely transfer and appropriate bed assignments for patients.
  • Provide accurate patient information to assist in developing initial care plans.
  • Maintain accurate data in Meditech and Central Logic systems.
  • Serve as a consultant and mediator for healthcare professionals regarding patient transfers.
  • Track and trend barriers to patient intake flow and develop action plans for improvement.
  • Demonstrate problem-solving, analytical, and critical thinking skills.
