About The Position

The Transfer Center Coordinator will ensure smooth system-wide patient placement, streamline transfer processes both into and out of all hospitals, and advance the seamless operation of clinical decisions related to patient movement. The transfer center coordinator will work with the patient placement nurse, room control associate, registration staff in collaboration with other departments and clinical leaders. Ensure optimized transfers both into and out of all hospitals across the system. Including internal transfers, upgrades in care and external transfer requests both into and out of the system. Includes working closely with care coordination, outside facilities, EMS partners and other community and statewide resources on a daily basis, as well as in times of disaster or diversions, over crowding or any other events as they may arise.

Requirements

  • Associate degree or bachelor’s degree from an accredited school of nursing
  • RN License. Licensed RN with an acute care background
  • Minimum of one (1) year of experience with acute care background
  • Working knowledge of Cerner EHR
  • Proven ability to lead cross-campus standardization efforts and projects involving multiple stakeholders
  • Excellent communication skills and ability to provide clear direction with supporting education to ensure ongoing adherence to workflows
  • Must be a quick learner of software systems if not already familiar, excellent communication skills, problem solving and critical thinking skills, and ability to lead large groups or large cross institutional efforts

Nice To Haves

  • Hands on use of Capacity Managements patient flow software
  • Transfer process or transfer center experience preferred
  • Familiarity with regulatory requirements such as EMTALA, CMS and MSDHS reporting, etc.

Responsibilities

  • Supports the usage of electronic health system software to facilitate all patient movement into, out of and within all hospitals, as well as direct admissions, transport team services, and all other requests for patient movement within the system
  • Train staff and maintain compliance with use of the EHR, decision trees and algorithms, workflows and clinical decision support materials used to facilitate patient movement and appropriate patient placement.
  • Provide support and oversight of patient flow nurses, room control associates, nursing supervisors and other registration staff as it relates to patient movement and placement
  • Provide real time data and status of all hospital facilities to leadership on an ongoing basis 24/7/365.
  • Generate actionable reports and analytics related to both internal and external workflows and maintain compliance with regulatory and reporting requirements
  • Provide leadership and oversight to maintain daily as well as monthly communication about all patient flow efforts, issues and concerns as they arise.
  • Collaborate with leaders across all campuses in clinical care, medical staff leadership, IT, care coordination, etc to ensure smooth system processes and technology driven patient movement efforts are successful and patient centric at all times
