Job Summary: Coordinates patient care needs by facilitating patient placement in the appropriate nursing unit utilizing pre-established criteria and consulting with members of the interdisciplinary team. Receives and processes requests for assistance with acute patient transfers and/or admissions of patients from medical facilities.
Requirements
- High School Education/GED or equivalent: Required

Nice To Haves

Responsibilities
- Performs telephonic patient coordination, discharge follow-up with physicians and patients and disease management programs.
- Coordinates with other clinics and hospitals assisting with physician consultation, transfer and coordination of patient ground/air transport.
- Collaborates frequently with Emergency Room (ER), and Post-Anesthesia Care Unit (PACU) and bed boards to keep abreast of possible admissions/transfers.
- Collaborates with the Bed Assignment/Staffing Group, Patient Placement Coordinator or Administrative Supervisor per shift to determine census and acuity.
- Collaborates to resolve bed availability challenges.
- Receives the admission orders and ensures that orders are sent to the appropriate nursing unit.
- Documents coordinator and patient interactions in electronic or hardcopy chart in a manner consistent with hospital standards.
- Creates reports from patient data such as volumes and outcomes.
- Performs other job-related duties as assigned.
