Brown University Health’s EpiCenter is the hub of capacity management and access across the system. It serves a vital role in managing patient flow and throughput at the system level and at each individual site, having visibility across all hospital operations and maintaining communication with all disciplines and teams. The EpiCenter is a central department with several branches charged with managing patient flow and throughput through access, capacity and flow, and transport teams with the primary goal of ensuring the right patient is in the right place at the right time. Under the direct supervision of the Clinical Manager (or designee) the Transfer and Access Navigator is responsible for triaging, coordinating, processing, and documenting patient transfers and transfer requests into Brown University Health. The transfer and access navigator will work collaboratively with the capacity and flow navigator, site-level nursing departments, and physicians to facilitate efficient movement of patients throughout the Brown Univ. Health system. The transfer and access navigator will need strong communication and problem-solving skills as she/he will serve as the clinical and administrative facilitator for said transfers in accordance with established departmental policies, algorithms and standard operating procedures.
