About The Position

The TPAC Coordinator is responsible for overseeing the management of the Transitional Post-Acute Care residence, ensuring a safe and effective transition for patients from acute care to home. This role involves assessing, planning, implementing, coordinating, monitoring, and evaluating care options and services, with a focus on developing comprehensive care plans that include both long-term and short-term goals for patient improvement.

Requirements

  • Must have a current, unencumbered RN license to practice in the state of Colorado (compact acceptable).
  • 2+ years of experience with MDS in a long-term care environment is preferred.
  • Must be able to relate information concerning a patient/resident's condition.
  • Able to follow all applicable policies and procedures as well as current federal, state, and local standards, guidelines, and regulations that govern swing bed facilities.
  • Must be able to react to emergency situations appropriately when required.
  • Subject to a pre-employment background check and drug screen.

Responsibilities

  • Coordinate Interdisciplinary Team's work along with the Resident Assessment Instrument process, ensuring the accuracy, timeliness, and completeness of the MDS and Interdisciplinary Care Plan.
  • Establish the schedule for completion of the Medicare required assessments.
  • Follow a multi-disciplinary model to identify patient needs and appropriate reimbursement.
  • Complete and/or oversee the completion of all Medicare MDS assessments.
  • Accurately code the MDS using information obtained via medical record review, observation, and interviews with facility staff, residents, and family members.
  • Participate in the admission and discharge process of prospective patients regarding their nursing needs and appropriate placement; facilitate problem-solving for complicated admissions.
