About The Position

Join our team as an Outpatient Total Joint Replacement Case Manager, where
you’ll lead care for orthopedic patients across a 90-day episode, ensuring
seamless transitions from pre-operative preparation through recovery. Under
general supervision, you’ll provide compassionate, professional communication to
patients and colleagues, triage calls per protocol, and coordinate inpatient
care as needed. Using the nursing process, you’ll develop individualized care
plans that address each patient’s physical, mental, and social needs, deliver
patient and family education, and collaborate with a multidisciplinary team to
arrange timely services and optimize outcomes. If you’re driven to improve

Here’s What You Can Expect

Requirements

• Licensure: Individual with active license to practice in the state of Iowa as a Registered Nurse (in good standing)
• Knowledge: Experience in case management and cross disciplinary support highly desired
• Communication skills. Ability to communicate calmly and professionally to patients and physicians.
• Curious. You practice the art of listening, asking clarifying questions, and avoiding interrupting your patient.

Responsibilities

• Adhere to Physician recommendations, coordinate care for patient needs and follow protocol related to a patient's condition
