About The Position

Under the direction of the Director of Residential, Clinical and Aftercare Services, the therapist will provide individual, family, group therapy and aftercare services to assigned residential and aftercare clients and their families, while corresponding and collaborating with outside agencies to support the clients’ goals and to maintain community and family connections.

Requirements

• Masters level social worker or mental health counselor, LMSW, LCSW, MCH, LMHC
• experience working with individuals, families and groups preferred
• recent Masters level graduates encouraged to apply
• ability to function as part of a multidisciplinary team
• valid driver’s license required

Responsibilities

• Develop comprehensive clinical assessments on assigned individuals and families
• Serve as the ongoing liaison with community agencies and referral sources
• Complete discharge summary and assist with discharge planning on each assigned case
• Provide aftercare services for assigned cases post-discharge
• Meet at least once a week with treatment team members in rounds and/or unit meetings to coordinate treatment planning.
