About The Position

The LISW or LMHC provides assessment, diagnosis, and therapeutic services to children, adolescents, and adults to promote optimum mental health within the integrated behavioral health model. Additionally, the counselor uses their unique social work skills and treatment modalities to meet the needs of the individual(s) needing services, including coordination of behavioral and medical health care and external resources.

Requirements

  • Master’s degree in social work or mental health counseling from an accredited university required.
  • 2-4 Years’ experience working with patients who have experienced trauma required.
  • Licensed Independent Social Work or Licensed Mental Health Counselor licensure in the state of Iowa.
  • Current BLS or CPR/First Aid certification
  • Mandatory reporter training for child and dependent adult abuse certificate
  • Strong analytical, critical, and problem-solving skills
  • Self-motivated to develop and maintain own competence as it pertains to the position.
  • Ability to work with diverse groups of people and with a large degree of independence
  • Ability to handle multiple tasks simultaneously and to deal with difficult situations
  • Ability to work with and through an interpreter
  • Understanding of and ability to diagnose using DSM 5
  • Knowledge of and how to access all area community resources
  • Skilled in program development
  • Working knowledge of MS Office Products (ex. Excel, Word, Outlook, Power Point) and Windows Platform. Must be comfortable moving within an EHR computer system regularly
  • Ability to respond calmly in stressful situations
  • Skilled in planning, organizing, adaptability, attentiveness to detail and flexibility of assignment.
  • Maintains strict confidence

Responsibilities

  • Provides psychosocial assessments of individuals and families to identify emotional, social and environmental strengths and challenges related to their diagnosis, medical condition, treatment, and/or life situation.
  • Formulates, develops and implements a comprehensive psychosocial treatment plan using appropriate clinical social work and/or counseling treatments and interventions. Interventions may include crisis intervention, brief and long-term individual, marital, and family and/or group therapies.
  • Provides therapeutic crisis intervention and emergency services as required.
  • Prepares and maintains a clinically appropriate patient behavioral health record in the EMR.
  • Provides education to patients and families around issues related to adaptation to the patient’s diagnosis, medical condition, treatment and/or life situation.
  • Maintains working knowledge of and liaison with community agencies and resources.
  • Provides consultative services to health care team members within scope of care definitions as needed.
  • Follows policies and procedures
  • Develops and maintains own competence.
  • Works to improve work processes and clinical outcomes including health disparity and quality improvement collaboratives.
  • Assists with the development of special grants, groups, and educational programs.
  • Maintains a safe working environment and practices safe working habits.
  • Assists in control of People’s Community Health Clinic resources.
