About The Position

Requirements

  • Master’s degree in social work, social sciences, or education (or a bachelor’s degree with relevant experience).
  • Experience in counseling, crisis intervention, and working with minority populations.
  • Experience working with diverse populations.
  • Valid driver's license and access to a personal vehicle for regional and statewide travel.
  • Awareness of cultural and socioeconomic characteristics of clients.
  • Ability to provide home-based supports, maintain professionalism, and work independently in challenging conditions.

Responsibilities

  • Maintain a caseload of up to six (6) IFP cases.
  • Conduct strength-based assessments to address safety risks and prevent out-of-home placements.
  • Deliver concrete and clinical interventions, skills training, and community resource referrals.
  • Use approved assessment tools and a solution-focused approach.
  • Attend weekly supervision meetings and bi-monthly Family Support team meetings.
  • Maintain timely and confidential electronic client records, adhering to documentation guidelines.
  • Collaborate with DCF through verbal and written updates and participate in monthly case reviews.
  • Assist with outreach efforts, such as presentations and mailings to referral sources.
  • Provide transportation for clients when aligned with treatment goals.
  • Participate in a 24/7 on-call rotation, offering crisis intervention for IFP and RTFT clients.
