About The Position

The Ohio Department of Behavioral Health provides in-house services to clients in the community who have been referred by Franklin County Community Treatment Teams. The role involves working with case managers to implement intervention plans, assessing client progress, observing behavior and symptoms, and providing mental health and supportive services for clients in various community settings. The position also includes assisting clients in acquiring and maintaining skills necessary for successful community transition, communicating with supervisors and fellow staff, engaging clients in conversation, ensuring a safe environment, and performing required documentation and miscellaneous duties.

Requirements

  • Formal education in arithmetic that includes addition and subtraction and in reading, writing, and speaking common English vocabulary.
  • Must possess a high school diploma or a certificate of high school equivalence pursuant to Ohio Administrative Code 5123:2-3-01(F)(1)(c).
  • For positions at Veteran Services, must possess STNA certification and/or be listed on the Ohio Nurse Aide Registry.
  • A valid driver's license is required for positions that require operation of a motor vehicle to transport consumers.

Responsibilities

  • Provides in-house services to clients in the community who have been referred by Franklin County Community Treatment Teams.
  • Works with the case manager to implement intervention plans addressing the problems which necessitated the referral.
  • Assesses clients' progress toward meeting treatment goals and documents the same.
  • Observes and monitors behavior and symptoms.
  • Provides mental health services (e.g., relapse prevention education) and supportive services (e.g., community adjustment activities) for clients residing in various community settings.
  • Assists/trains/participates in direct care and supervision to clients to help them identify, understand, and acquire and maintain necessary skills for successful community transition.
  • Communicates pertinent information to appropriate supervisor and fellow Home-Based Support Service (HBSS) staff members regarding client's progress, needs, status, and activities.
  • Engages clients in conversation and provides for a safe environment.
  • Accompanies clients as needed to activities, appointments, and community-based services; implements appropriate therapeutic precautions.
  • Performs charting and required documentation; attends and participates in meetings and mandated trainings.
  • Performs a variety of miscellaneous duties such as light housekeeping, identifying and reporting safety concerns, and providing clerical support.
