About The Position

Under the direction and supervision of the program director, the Pathways to Success Therapeutic Mentor, will provide youth with positive role modeling to help them learn better social skills, interpersonal skills, problem solving strategies, or daily living skills.   Therapeutic Mentors will model positive ways to interact with other children and adults in the community to assist the youth with developing healthy relationships and develop social confidence. Therapeutic Mentors help children and teenagers build on their strengths while working in collaboration with the child’s family and their clinical team to achieve a specific skill-building goal.

Requirements

  • Excellent interpersonal skills, cultural competency, and experience with diverse populations
  • Strongly self-motivated
  • Strong communication skills
  • Teaching skills, non-judgmental personality, and empathic listening skills
  • Ability and knowledge of basic computer programs for service documentation and report writing
  • High school diploma or GED
  • Must have a valid driver’s license in the state of Illinois
  • Attend initial and ongoing required training (provided on site)

Responsibilities

  • Assisting the youth with developing positive social skills with other children and adults
  • Assisting the youth with skill building to help increase positive communication, emotion regulation and participating in healthy relationships
  • Providing the youth with positive reinforcement to build confidence
  • Providing Pathways to Success mentoring services face to face, over the phone, in the office, in the home, or community setting
  • Improving the youth’s connection and engagement within their community
  • Educating, modeling, and motivating the youth to increase their involvement with others in their community
  • Supports the youth and family during times of crisis
  • Focusing on a strength-based approach
  • Assisting the youth in self-advocacy
  • Identifying and providing information about the program-enrolled youth’s needs and strengths
  • Identification and building of natural supports
  • Setting goals, designing, implementing, and evaluating outcomes of programs
  • Works closely with a multidisciplinary team
  • Responsible for the timely submission of Medicaid mental health billing
  • Provide timely documentation and reporting under the guidelines and regulations of Trinity Services standards
  • Participates in weekly team meetings
  • Traveling from various locations within the community to provide the family and youth with supports
  • Perform other duties as assigned
