About The Position

Since 1958, United Counseling Service (UCS) has been dedicated to providing exceptional care and support to individuals and families in our community. Through a wide range of programs, services, and educational opportunities, we strive to meet the diverse needs of those we serve. Our team includes professionals from various fields, including administration, clinical services, nursing, education, case management, psychiatry, and direct support, working across 17 facilities throughout Bennington County. As a proud affiliate of Vermont Care Partners—a statewide network of 16 non-profit community-based agencies—we deliver comprehensive mental health, substance use, and intellectual and developmental services across Vermont. We are currently seeking compassionate, motivated team players to join us in making a positive impact and building a stronger community. The Therapeutic Case Manager provides direct services to children. Allows easy access, timely response, and flexibility to deliver skill building services in the community.

Requirements

  • High School diploma or equivalent with experience providing supportive services to children.
  • Valid Driver’s License

Responsibilities

  • Provides access to community services and activities to children with the intent of teaching, practicing, and mastering skills to decrease difficulty and increase success in the community. Services are to be provided in groups or individually.
  • Initiates and maintains, in a timely and thorough manner, the wide variety of record keeping and administrative documentation required to account for services provided to consumers – including, but not limited to, treatment plans, progress notes, CANS, and time sheets. Follows all UCS record-keeping procedures and performs other duties as necessary. The Case Manager is responsible for the chart management of clients that are receiving therapeutic, not clinical, services through UCS.
  • Attends all UCS and community meetings necessary to promote coordination and communication.
