About the position

Under the direction of the Community Relations Coordinator, in conjunction with the Deputy Director and Site Medical Director, the Peer Worker provides health education, and health promotion services to address barriers to care and to ensure that residents in need of care in the Chelsea-Clinton community as well as other areas as needed are all identified, and services provided. This work is carried out in support of the mission and goals of Ryan Chelsea-Clinton.

Responsibilities

  • Provide patient education and health literacy on health, nutrition, exercise and other interventions that promote whole person wellness.
  • Develops, implements, monitors, and reassesses activities to perform during in reach and outreach to the community that communicate the roles of the care team at RCC medical practitioners, behavioral health providers, and other self-management aides while actively engaging the community.
  • Collaborates and consults with the Community Relations Coordinator, in identifying service areas in need of education and as identified by the needs assessment have chronic conditions that the medical providers at RCC can assist with.
  • Informs residents of catchment area of relevant benefits and entitlement programs and health insurance programs through the New York State of Health Marketplace.
  • Conducts intensive outreach to residents that have disengaged or are at risk of disengaging from care, and individuals whose psychosocial stressors are limiting their success in achieving healthy outcomes.
  • Develops linkages between community agencies and resources to ensure continuity of care.
  • Assists residents in enrollment of eligible insurance plans and financial assistance programs to improve access to Center services.
  • Attends required meetings, case conferences, supervision, trainings, Center events, and professional conferences as assigned.
  • Immediately report any problems or unusual occurrences to the Community Relations Coordinator, Deputy Director, or appropriate site administrator.
  • Special projects and other duties assigned.

Requirements

  • 2 years of relevant experience working with a population with chronic conditions in a medical setting.
  • Effective communication and writing skills.
  • Strong organizational skills and attention to detail.
  • Strong skills in Microsoft Word, Excel, and Outlook.
  • Bachelor's degree or related field experience.

Nice-to-haves

  • Bilingual Spanish/English.
