Personal Service Coordinator / Intensive Case Manager (Temporary) 35 hours per week / $24.00 per hour (The actual compensation will be determined based on experience and other factors permitted by law.) Summary: Provide high-quality Intensive Case Management Services (ICMS) to clients eligible for permanent housing through the Department of Health Services (DHS) Housing for Health (HFH) program. Provide a client-centered approach and excellent customer service, sensitive to the challenges that people experiencing homelessness with a range of medical and behavioral health issues face as they move into and maintain permanent supportive housing.
